To become self excluded you will need to attend an interview with an AHA (Vic) officer.  At the interview your photo will be taken, and you will be required to sign the Deed of Self Exclusion. You may view a sample copy of the Deed here.  Please ensure that you read the draft Deed carefully and if required, seek independent advice concerning the contents of the document. The contents of the Deed can be further explained to you at the interview. You may bring any person or material you wish to the interview. When you call to arrange an interview time, you will be asked to nominate by name the venue/s you wish to be self-excluded from.
